Data collected and use of data

Identity (e.g. name, date of birth) Patient identification, appointment records, invoicing, accounting and tax records.

Communication (e.g. telephone numbers, email address, postal address)

  • Used and collected to communicate with the Patient

Health and medical history

  • In relation to the health history of the patient and to help treat the patient. Aids explanation of decisions made in respect of the treatments given/ proposed to patient, referrer or funding body.

Relating to the findings/ treatments/ decisions made/ outcomes in respect of treating the patient . 

  • To help treat patient, to explain decisions made in respect of the treatments given/ proposed to patient, referrer or funding body.

 

No racial, ethnic, religious, or sexual-orientation data are held.

 

Sharing of data

The data is being principally used for the reasons set out above. However, as a consequence of your treatment, the data will also be used to:

  • Communicate with you
  • Communicate with other health specialists in relation to your treatment (e.g. your hospital consultant)
  • Communicate with any funding bodies (e.g. BUPA) that support your treatment (e.g. to secure further funding).

These are the only reasons that your data will be shared. In the unlikely event that another reason to share data did arise, this would be discussed with you first and your consent obtained.

 

Data storage

  • Data relating to patient treatment and contact details will be kept securely.
  • Under normal circumstances, all patient data, including data relating to patient care will be destroyed/ deleted in the most appropriate manner under the following circumstances:
    • Once treatment has been completed and a period of eight years has elapsed from the date of the final treatment. The eight year period aligns with Records Management Code of Practice and allows reference to the success of previous treatment approaches should the same problem recur.
    • At the request of the patient.

 

Subject Access Request

  • Patients may request a copy of their records. Where a request is received, copies of the data will be sent to the patient within 1 month of the access request being received.
